Service Description
Provides primary care services on a sliding fee scale for uninsured Arizona residents with family incomes of 200% or less of federal poverty guidelines & who meet the eligibility requirements of the Tobacco Tax Primary Care Program.
Hours: Clinic hours: Monday - Friday 8:30 am - 4:30 pm
Afterhour emergencies: Go to nearest emergency room.
